I'm not much of an event guy, but the information looks like it matches up with the TRU event. You didn't note if it had a ribbon or not, but if it has a Classic Ribbon attached to it, it looks fine

The gender is a reference to the OT gender, not the gender of the pokemon itself. So when he says male, he means the person who caught it was playing as a male character. This information is more useful for events since in game pokemon cn be caught by any gender player, but it is still part of the form on the OP so most people include it as well.
